Diabetes Community Coordinator | Kaiāwhina Mate Huka

Nelson Central, Tasman

About the role | Mō te tūnga

We are seeking enthusiastic applicants wanting to offer support to people with pre-diabetes and diabetes, and their whānau.  

They will work with the wider team of Health professionals, community health workers, allied health & social workers, the Māori and Population Health team.

A personal experience with diabetes, either lived or whānau would be an advantage.  They will provide lifestyle education to patients and their whānau. Training will be provided by health professional colleagues and Diabetes New Zealand.

The Diabetes Community Coordinator | Kaiāwhina Mate Huka will administrate and co-facilitate diabetes self-management education programmes with health professionals. Programme specific training will be given for this. Completion of training in adult education would be an advantage. 

This person will ideally have links to community groups and providers or be able to develop them.

The Diabetes Community Coordinator will be located in Waimeha/Richmond and will also work to support whānau living in Whakatū/Nelson & Motueka. This role will usually be Monday to Friday but may be required to do some work outside of usual business hours on occasion and only as agreed. 

The Diabetes Community Coordinator | Kaiāwhina Mate Huka role is that of a Health Coach. The successful candidate will receive formal training for this role.

The successful applicant will have 

  • a full clean New Zealand driver's licence

  • comfortable with the use of Te Reo Māori

  • strong computer skills using Microsoft office and other programmes, a ‘can-do’ attitude and be able to demonstrate strong personal and professional boundaries

  • demonstrated commitment to the Treaty of Waitangi and culturally safe practice

The people in this role will be working in partnership with Te Piki Oranga.

Volunteer: Diabetes Support Group Coordinator - Tauranga

This role involves coordinating and facilitating support groups, organising morning tea, and engaging guest speakers. Support and training will be provided as necessary. The successful candidate will have a sound knowledge and understanding of diabetes – this may be through lived experience, or health care training. Diabetes New Zealand Tauranga Branch is part of Diabetes New Zealand Charitable Trust. We provide information, education and support to people and whanau living with diabetes.

Commitment

Regular - more than 6 months

Time required

3-4 hours per month

Reimbursement

Travel costs may be reimbursed by arrangement on an individual basis
